Sliding Fee Discount Program Policy

PURPOSE

To ensure all patients have access to care regardless of ability to pay, ObGyne Birth Center for Natural Deliveries (rebranding as BirthEzy) maintains a sliding fee discount program that aligns with NHSC requirements.

Discounts are based on family size and income using the most current Federal Poverty Guidelines (FPG).

No one will be denied access to services due to inability to pay.

Eligibility

- Patients may apply at any time; eligibility is reviewed annually using proof of income and family size.
- Discounts are available to patients at or below 200% of the Federal Poverty Guidelines (FPG).
- A nominal fee is charged to patients at or below 100% of the FPG.

Hardship Waivers:

If you cannot pay the nominal fee, you may request a hardship waiver. Essential services will never be denied.

This policy is publicly posted and explained to all patients during the intake process.

Multilingual Patient Notice (Required Statements)

English: No one will be denied access to services due to inability to pay. A discounted/sliding fee schedule is available based on family size and income.
Español: Nadie será negado a recibir servicios por no poder pagar. Un programa de tarifas con descuento/escalonadas está disponible según el tamaño de la familia y los ingresos.

हिंदी: किसी को भी भुगतान करने में असमर्थ होने के कारण सेवाओं से वंचित नहीं किया जाएगा। परिवार के आकार और आय के आधार पर रियायती/स्लाइडिंग शुल्क उपलब्ध है।
Français: Personne ne se verra refuser l’accès aux services en raison de son incapacité à payer. Un barème dégressif est disponible selon la taille de la famille et le revenu.

العربية: لن يُحرم أي شخص من الحصول على الخدمات بسبب عدم القدرة على الدفع. يتوفر جدول رسوم مخفضة/متدرجة بناءً على حجم الأسرة والدخل.
Русский: Никому не будет отказано в услугах из-за невозможности оплатить. Существует скидочная/скользящая шкала оплаты в зависимости от размера семьи и дохода.
